The BrookSide Assisted Living Community includes assistance with one to two supportive services daily. These supportive services may include, but are not limited to, assistance with daily living (ADL's) such as: personal hygiene, dressing, grooming, and assistance with mobility.

When there is a need beyond these basic supportive services, a small additional fee will be applied as follows:

Levels of Service:

	Level 1
	Two Supportive Services (included in the monthly rent)
	Level 2
	Includes assistance with 3-4 ADL's
	Level 3
	Includes 5-7 ADL's
	Level 4
	"Special Care" - includes extensive assistance or totally dependent care.
